Twelve places where ageing happens — and where you can push back.
Most pages about the hallmarks list twelve intimidating terms and leave you feeling smaller than when you arrived. This one does the opposite. Twelve doors into the machinery of ageing, each with something behind it you can actually do — sometimes a great deal, sometimes very little, and this page says which.

Entry point. That is the whole idea. Not twelve separate diseases — twelve places where the machinery can be reached.
Why wrinkles are not one.
a hallmark of aging should accompany the aging process and aggravate with time. That’s trivial. Wrinkles will do that, but they are not a point of your body that you can intervene on to improve aging. You just improve cosmetics.Guido Kroemer
- It gets worse with age. Necessary, and nowhere near enough on its own.
- Push it harder and ageing speeds up. Accentuate it in the laboratory and age-related disease arrives earlier.
- Ease it and ageing slows. The hard one — slow the hallmark and you slow, halt, or at best partly reverse an aspect of ageing.
And the honest part. Kroemer is plain that the effects are always small — not ten or fifty per cent. What matters is that a small push on one hallmark moves the whole system, which is exactly what shows they are connected.
One circle, twelve slices. That is the whole idea.

Genomic instability

Damage accumulates in the DNA faster than the repair systems clear it, and some of it becomes permanent.
Every cell is copying three billion letters, over and over, under attack from sunlight, chemistry and its own metabolism. Mistakes are normal; the repair crews are very good. Ageing is what happens when the mistakes outpace the crews.
Not much directly, and be suspicious of anyone selling you otherwise. What is well established is what causes the damage: tobacco, too much sun, heavy alcohol, and the diagnostics that catch the consequences early.

Telomere attrition
The lace ends fray

The protective caps on the ends of chromosomes shorten each time a cell divides. Past a certain point the cell stops dividing or turns senescent.
A shoelace has a plastic tip. Without it the lace frays and becomes useless. Telomeres are that tip, and every division trims a little off.
Honestly: less than the supplement shelf implies. Nothing safely and reliably lengthens human telomeres. Smoking, obesity and long-running stress all track with faster shortening, so the lever is the life, not a pill. And longer is not automatically better — switching telomerase back on is also how a cancer makes itself immortal.

Epigenetic alterations
The instructions get misread

The chemical marks that decide which genes are switched on drift with age, so cells begin doing the wrong job for the tissue they are in.
Every cell carries the same book. The epigenome is the set of bookmarks telling a liver cell to read the liver chapters. With age the bookmarks slide, and cells start reading the wrong pages.
This is the hallmark the frontier is most excited about, because partial reprogramming has reversed it in animals. In humans it is not a treatment yet. What you can do today is the ordinary set — sleep, exercise, not smoking — which tracks with slower epigenetic ageing in the clocks that measure it.

Loss of proteostasis
Proteins stop folding properly

The systems that fold proteins into the right shape, and dispose of the ones that fold wrongly, become less reliable, so misfolded proteins accumulate and clump.
A protein is a long ribbon that only works when folded into one exact shape. Ageing is partly a folding problem, and the clumps it leaves behind are what a pathologist finds in an ageing brain.
Exercise and heat exposure both switch on the cell’s own repair chaperones. Sleep matters here more than it gets credit for: the brain’s overnight clearance is when a good deal of this disposal happens.

Disabled macroautophagy
The clean-up crew slows down

The cell’s recycling system — breaking down its own damaged parts so the material can be reused — becomes less active with age.
Your body’s clean-up crew. It takes old, broken cell parts, breaks them down and builds new ones from the pieces. When it slows, the debris accumulates, and that accumulation is a large part of what ageing looks like from the inside.
Going without food for a stretch is the strongest everyday lever — time-restricted eating or a longer fast. Hard exercise does it too. Coffee, green tea and olive oil each nudge it. Rapamycin is the most convincing drug and remains prescription-only and under study.

Deregulated nutrient-sensing
The fuel gauge stops reading true

The network that reads how much food is available — insulin, mTOR, AMPK, the sirtuins — stops responding accurately, so the cell keeps building when it should be clearing.
A body that always thinks food is plentiful never switches into maintenance mode. Growth and repair are opposites, and ageing is partly the gauge jamming on "plenty".
This is the most actionable hallmark on the list. Eating within a shorter window, keeping refined carbohydrate down, staying strong and lean, and moving after meals all move it. Metformin has mouse evidence that has not yet been demonstrated in humans.

Mitochondrial dysfunction
The power stations falter

The organelles that make the cell’s energy become fewer, leakier and less efficient, and the damaged ones are cleared more slowly.
Mitochondria are where being alive is paid for. Tiredness that is not sleepiness is often this. So is a good deal of what people call feeling old.
Exercise is the only intervention that reliably builds new mitochondria — endurance work especially, and interval work most efficiently. No supplement comes close, whatever the label says.

Old cells refuse to leave

Damaged cells stop dividing but do not die. They stay, and they broadcast inflammatory signals to the tissue around them.
A retired worker who will not leave the building, and who complains loudly to everyone still working. A few are useful — they stop damaged cells becoming tumours. Too many, and the whole tissue ages faster.
Exercise clears some. Senolytics — drugs that persuade these cells to die — are the most watched frontier here, in human trials, not on the shelf. Anything sold today as a senolytic supplement is ahead of its evidence.

The reserve of cells that replaces worn-out tissue shrinks and becomes less responsive, so healing slows and tissues thin.
Every tissue keeps apprentices in reserve to replace the workers who wear out. With age there are fewer apprentices and they are slower to answer the call. It is why a cut heals slower at sixty than at sixteen.
Nothing you buy. What protects the reserve is the unglamorous list — sleep, protein, resistance training, not smoking. Stem cell clinics selling injections for ageing are, at the time of writing, selling ahead of the evidence.

Altered intercellular communication
Cells stop talking properly
Auf der Map · 03 Radical Life ExtensionThe signalling between cells and between organs — hormones, immune messengers, nerve signals — becomes noisier and less coordinated.
A body is an organisation, and ageing is partly a communications failure inside it. Departments stop hearing each other, and each starts optimising for itself.
Kroemer’s point applies most here: everything is connected. Moving any other hallmark moves this one. Directly, the levers are sleep, daylight, and keeping the inflammatory load down.

A fire that never goes out

A low, persistent, body-wide inflammatory state that rises with age even without infection — often called inflammaging.
Inflammation is how a body fights and repairs; it is meant to flare and then stop. This one never stops. It is quiet enough that you do not feel it, and loud enough to damage tissue for decades.
Sleep, weight, muscle, oily fish, and treating gum disease and other quiet infections. This is also the hallmark most visible in an ordinary blood test, which makes it one of the few you can actually watch.

The community of microbes in the gut loses diversity and shifts toward species that provoke inflammation, while the gut wall becomes more permeable.

Variety of plants over any single superfood, fibre, and fermented foods. Antibiotics when you need them and not when you do not. Probiotic capsules are sold far ahead of what the trials show.
